Metallica, étant connu pour les expérimentations, a créé quelque chose d'inhabituel avec ce neuvième album. « S&M » a été enregistré pendant deux concerts à Berkeley avec l'Orchestre Symphonique de San Francisco. C'est pourquoi ils ne se sont pas limités à des titres plus lents, mais ont également travaillé sur des titres plus agressifs comme « Hero Of The Day » ou « Master Of Puppets ». Le nom « S&M » est un jeu de mots sur l'abbréviation du sadomasochisme. Dans le contexte de l'album, ces lettres sont les abréviations de « Symphony » et « Metallica ». Ce jeu de mots est reflété par la pochette sur laquelle le « S » est représenté par une clef de sol inversée et le « M » par le « M » de Metallica.
